The Thompson Test (calf squeeze test) is a fast clinical assessment used to diagnose a complete Achilles tendon rupture. When a provider squeezes the gastrocnemius-soleus complex, the foot should automatically point downward (plantarflexion). If the foot remains completely still, it indicates test is positive.

Note: Patients can sometimes still weakly flex their ankle using deep secondary muscles, so a lack of movement during the squeeze test is the true warning sign
