🧾 How to Create a Google Form

📱 On Mobile (via Browser or Google Forms app)

1. Open your browser (like Chrome).
2. Go to 👉 forms.google.com
3. Sign in with your Google account.
4. Tap the “+ Blank” button (➕) to create a new form.
5. Type your form title and description.
6. Add questions by tapping the “+” button.

Choose question type (multiple choice, short answer, checkbox, etc.).
7. To make a question required, toggle the “Required” switch.
8. Tap the palette icon 🎨 to change theme or color.
9. Tap the eye icon 👁️ to preview the form.
10. When done, tap the send icon (✉️) → choose link (🔗) → copy or share it.

🔹 Step 1: Open Google Forms

1. Go to 👉 https://forms.google.com
2. Sign in with your Google account.
3. Click “Blank Form” (➕).

Description: “Please fill out the form below to complete your registration.”

1. Full Name → Short answer (Required ✅)

3. Phone Number → Short answer (Required ✅)
4. Date of Birth → Date
5. Gender → Multiple choice (Male / Female / Other)
6. Address → Paragraph
7. Course / Event / Program Name → Dropdown or short answer
8. Preferred Time Slot → Multiple choice or dropdown
9. Upload Photo or ID Proof (if needed) → File upload
10. Declaration / Agreement → Checkbox (“I confirm that the above information is true.”)

Click the 🎨 palette icon to change the theme or color.

Click the ⚙️ Settings icon → enable “Collect email addresses” (optional).

Toggle “Required” for all important questions.
