The Pressure: Briefly name the obstacle.
The Pivot: Explain the decision you made to solve it.
Example: Interviewer: "Tell me about a time you failed." You: "I cost my team three weeks on a launch because I misread a vendor's timeline (The Point). The pressure was intense because we had a hard deadline with the CEO (The Pressure). I decided to halt production immediately to re-negotiate the contract rather than trying to 'fix' a broken path. That decision saved the project and became our new standard for vendor vetting (The Pivot)."
