How to start Far Harbor:
1. Do Nick Valentine missions
2. Go to Nicks' office in Diamond City
3. Start a new side quest about Nakano Family Troubles
4. Go to North past Mahkra Fishpacking factory out side the map area
5. There you will find a new house and the quest to Far Harbor starts
