Upper Jaw Reconstruction Surgery for Dental Implant Surgery

This young man had developed a swelling and pain in his right upper jaw a year ago. He had presented to a hometown hospital for management of his condition. Imaging studies had been obtained, which revealed a radiolucent lesion in that region.
Realizing that this could be a cyst or a benign tumor, the doctor had counselled him and referred him to our hospital. Imaging studies and a biopsy were obtained, which revealed a diagnosis of ameloblastoma of the upper jaw.
There was widespread bony destruction evident in the imaging studies. Treatment planning would involve enucleation of the ameloblastoma. This would be followed by jaw reconstruction surgery. Dental implant surgery would be the final step towards complete rehabilitation.
The patient then underwent enucleation of his ameloblastoma. Teeth overlying the lesion were extracted. There was no evidence of recurrence of the benign tumor on follow up six months later.
He was then scheduled for upper jaw reconstruction surgery. Rib bone grafts were harvested to be used for filling in the bony defect left behind by the ameloblastoma. A Valsalva maneuver was performed to confirm patency of the thoracic cavity.
A flap was then raised at the region of the right upper jaw reconstruction. The rib bone grafts were used to restore the original form of the upper jaw. These were the fixed in place using titanium screws.
The patient was instructed to return in six months for dental implant surgery. There would be complete consolidation of the bone grafts with the alveolar bone. Dental implants would then be fixed on the consolidated upper jaw bone. Crowns would be fixed on the dental implants in six months to complete his rehabilitation.
