What is a MELD score and how is it calculated?
In this video, we show you how to calculate your MELD score for free using LiverTracker's online MELD Score Calculator — no sign-up, no download, no account needed.MELD stands for Model for End-Stage Liver Disease. It is the scoring system used by every transplant center in the United States to determine liver transplant priority.
Your MELD score ranges from 6 to 40 — the higher the number, the more urgent your need for a transplant.But here's what most patients don't know: there are actually THREE versions of MELD, and the same lab values can produce very different scores depending on which version is used.
In this demo, the same patient scores 18 on Classic MELD, 23 on MELD-Na, and 25 on MELD 3.0.

MELD score is used to estimate how urgently a patient needs a liver transplant. It is calculated from routine blood work — specifically bilirubin (liver waste processing), INR (blood clotting), and creatinine (kidney function). Transplant centers rank waitlisted patients by MELD score. A higher MELD means higher priority for receiving a donor liver.WHAT ARE THE THREE MELD VERSIONS?
Classic MELD (since 2002) — Uses bilirubin, INR, and creatinine. Score range: 6–40.
MELD-Na (since 2016) — Adds serum sodium to better predict 90-day mortality in patients with portal hypertension.
MELD 3.0 (since 2021, NIH update) — Adds gender and albumin with interaction terms. Corrects the known sex-based disparity where creatinine underestimates kidney dysfunction in women. This is the version currently used by UNOS for transplant allocation.
NIH-STANDARD FORMULAS USED IN THIS CALCULATOR:
MELD = 3.78 × ln(Bilirubin) + 11.2 × ln(INR) + 9.57 × ln(Creatinine) + 6.43
MELD-Na = MELD + 1.32 × (137 − Na) − 0.033 × MELD × (137 − Na)
MELD 3.0 = Includes female sex coefficient (+1.33), serum albumin, and bilirubin-sodium / albumin-creatinine interaction terms

6–9: Low risk — 1.9% three-month mortality
10–19: Moderate — 6% three-month mortality, closer monitoring needed
20–29: High risk — 19.6% three-month mortality, transplant evaluation
