Prior to the guideline revisions of 2021, the table of risk selection was made based on a treatment plan that included prescribed medication. The new guidelines for use of column three in the MDM chart should be selected based on the risk of managing the patient on a specific treatment plan. Moderate level risk notes that prescription drug management is an example of moderate risk. Therefore, when auditing E and M services, if the documentation demonstrates that the rendering provider is managing the patient on a specific prescription drug then RX management is supported. Inclusion of instructions for use and strength of medication are recommended, but should not be the determining criteria for the level of risk, unless the risk varies due to off-label recommendations of use. E/M Guidelines do NOT include best documentation practice, but the minimum necessary. A valid audit must also require minimum necessary, while educating to best practices.
