Kandu V & Kenau Attack Trainer (4/3/87)
On March 4, 1987, 21 year old orca trainer Jonathan Smith was attacked at SeaWorld San Diego by wild-caught females, Kandu V and Kenau.
During a show, one of the females suddendly grabbed Smith after he fell off her back, and dragged him to the bottom of the pool. She then carried a bloody Smith back to the surface and spat him out. Throughout the entire incident, Smith continued to smile and wave at the crowd, despite the danger he was in.
As other trainers are on the slideout attempting to regain control of the whales, one slams into Smith, submerging him yet again.
The females continue to push and drag Smith around, repeatedly bringing him to the bottom of the show pool. The above video, which doesn't show the entire incident, shows Smith being dragged underwater 5 times.

Fortunately, Smith managed to escape alive, though he suffered from a ruptured kidney, sustained a 6-inch laceration to his liver, and his torso was covered in bites and scrapes.
Later reports of the incidents indicate that the two whales involved were ~10 year old Kenau, and ~9 year old Kandu V. This 1987 incident was not an isolated one for the whales; both females had been involved in other incidents throughout their time in captivity, and were both known to be aggressive and unpredicatble. The following includes incidents both whales were confirmed to be involved in:

On two seperate occassions in 1983 both whales took trainers in their mouths and momentarily refused to let go, as well as relentlessly pushing trainers from one end of the tank to the other.
February 23, 1984: Kandu V took trainer Joanne Hay in her mouth and pinned her against the wall during a show.
November 1984: Kandu V briefly grabbed the legs of trainer Georgia Jones during a show, but released the trainer unhurt.
November 1986: Kandu V pressed her snout against trainer Mark Beeler and held him against a wall during a show.
June 15, 1987: Kandu V landed ontop of Webber and pushed her to the bottom of the pool during a training session. Webber suffered from a broken neck.
