You will need:

Bacardi (or White Rum... Coconut Rum is fine for a substitute)
Coconut Syrup (or Puree)
Pineapple Juice
Milk
Single Cream
Pineapple (optional)
Ice

Cocktail Shaker
Measurer
Strainer
Afternoon of doing nothing else
