During the course of the quest to retrieve the ashes, you’ll run into a group of Dragon cultists led by a man named Kolgrim. They believe that Andraste has been reincarnated as a dragon, which currently lives on the mountain. This jabroney will ask you to dump a vial of dragon’s blood into the urn of sacred ashes… since him and his cultist buddies haven’t been able to get through the traps and puzzles in the temple.

If you poison the urn while Leliana and / or Wynne are in your party then they will attack you, forcing you to fight them to the death. However, if Leliana was hardened during her personal quest prior to destroying the urn, then you can use an intimidate speech check to convince her to be chill and let it slide.

Even if Leliana dies, she will return in Dragon Age II and Inquisition with a flimsy explanation of how she survived (it gets REALLY weird in the end slides of the Trespasser DLC)

But what about the Arl? Can Arl Eamon die during the Urn of Sacred Ashes quest? No, he's got enough plot armor to keep him alive no matter what choices you make with the urn and his son Connor. You cannot progress the main story until you've revived Eamon. Your character will automatically grab a pinch of the ashes, even if you destroy the urn.
