We need your help to improve discharge planning & Home Healthcare. At Aaidebook we are focused on working with hospitals and their Hospital Reduction Readmission Program (HRRP) initiatives and have developed forward thinking solutions to assist. Readmission cause patients to lose confidence in healthcare and hospitals to be penalized on their reimbursement rates. In a time where healthcare has proven to be essential for humanity we cannot afford either scenario.

Have you ever wondered if one of your patients received the followup care they needed? What about if they picked up their medication and stuck with their discharge plan. I had a similar questions when I worked in hospitals. I would ask myself why does this patient keep coming back and who is looking after them when they leave. These questions inspired me to found Aaidebook the innovations lease in home health care. Our mission is to reduce hospital readmission and improve patient prognosis. We do this by using technology and proven teamwork focused case management.

Right now we are working on a new tool that is designed to bring everyone involved in a patients care together where we all can share and receive the patients progress from the moment they leave the hospital. This platform is conveniently named going home and here is what you can do to help.
We are looking for motivated health care providers to share their experience and views on discharge planning, homecare, and readmission reduction procedures at their work place.
