Fetoscopic Endotracheal Occlusion (FETO) to Treat CDH

Congenital diaphragmatic hernia (CDH) presents a significant challenge in neonatal care, characterized by an opening in the diaphragm that allows abdominal organs to move into the chest. This condition interferes with lung development and cardiovascular system maturation, potentially leading to pulmonary hypoplasia and hypertension in newborns — complications that are often life-threatening.

As fetal interventions for treating severe CDH have been limited over the years, Chris Derderian, MD, pediatric and fetal surgeon at the Colorado Fetal Care Center, reviews the concept of fetal tracheal occlusion and introduces recent findings from the New England Journal of Medicine TOTAL trial. This experimental procedure, now known as fetoscopic endotracheal occlusion (FETO), is designed to promote lung growth by temporarily obstructing the fetal airway. It has evolved and continues to be a promising treatment option for select families.
