Two safety failures in one afternoon on USS Hue City. The Navy's response was yelling at me for taking lunch.

In 2004 I was an undesignated seaman waiting for OS A-school. Deck division almost killed me when a boatswain's chair knot failed and my safety vest didn't inflate. This is what happened next, and why the chow line is the part you have to understand to know what deck division actually is.
