Benign Prostatic Hyperplasia (BPH)

Definition:
Non-cancerous enlargement of the prostate gland, usually in men over 50, causing lower urinary tract symptoms (LUTS) due to urethral compression.

Cause:
Driven by Dihydrotestosterone (DHT); hyperplasia of stromal and epithelial cells in the transitional zone.

Symptoms:

Obstructive: Weak stream, hesitancy, straining, incomplete emptying

Irritative: Frequency, urgency, nocturia

Diagnosis:

DRE: Smooth, enlarged prostate

PSA: May be mildly raised

Ultrasound: Prostate size, residual urine

IPSS Score for symptom grading

Treatment:

Mild: Watchful waiting

Moderate-Severe:

Alpha-blockers (Tamsulosin)

5-alpha-reductase inhibitors (Finasteride)

TURP if medical therapy fails

Complications:
Urinary retention, UTIs, bladder stones, hydronephrosis (rare)
