So we're talking about blisters under the forefoot -- under the metatarsal heads. Most often they'll occur under the first met head or the central met heads.

Blisters under the ball of the foot are caused by something called shear. Shear is the stretching of skin and soft tissue. As the metatarsal head moves one way, the force of friction opposes this and keeps the surface of the skin stationary. The result is all the soft tissues in between are stretched.

Shear is normal and it happens with every step you take. In fact, the skin of the feet can tolerate a lot of shear. But when it becomes excessive and repetitive, the connections between the layers of skin can stretch too and sustain micro-tears. This happens in the prickle layer of the epidermis to form a fluid-filled blister. A blister can take a few days to a week or more to resolve.
