Since everything is happening inside his mind palace, all the action is basically just Sherlock's thoughts. For example, in the waterfall scene, Moriarty tells him "I am your weakness," and John is the one who comes out and saves him. SHERLOCK IS LITERALLY IMAGINING JOHN SAVING HIM.
