How is the ACL typically injured?

The ACL is the most commonly injured ligament in the knee.

To better understand how the ACL is injured, we need to understand what the ACL does. The ACL’s main responsibilities are limiting forward movement of the shin bone (tibia) and excessive inward rotation of the knee.

Most ACL injuries occur during non-contact acceleration or deceleration with an inward collapse of the knee (dynamic knee valgus). During this collapse the knee inwardly rotates excessively, stressing the ACL. This repetitive stress wears the ACL down and can eventually lead to injury.

Dynamic knee valgus can occur for multiple reasons but is often seen due to limited hip range of motion and/or weakness in the muscles of the trunk, hip, knee, or foot/ankle.

It is important to take a full body approach when rehabbing the ACL, not just focusing on the knee. We need to treat the body as a system because an injury at the knee is rarely because of something just at the knee.
