Difference between Modic changes & Hemangioma in vertebral body. Update on How to read MRI of spine

Modic changes and vertebral hemangiomas are two distinct conditions that can affect the vertebral body, and they have different characteristics and implications:

Modic Changes:

Modic changes, also known as Modic vertebral endplate and marrow changes, are radiological findings observed in the vertebral bodies of the spine on magnetic resonance imaging (MRI).
Modic changes are typically classified into three types: Type I, Type II, and Type III.
Type I Modic changes involve the replacement of normal vertebral bone marrow with fatty tissue. They are often associated with acute or subacute vertebral endplate fractures, inflammation, or infection.
Type II Modic changes involve a conversion of normal bone marrow to a more fibrous tissue. They are often seen in degenerative disc disease and are considered a sign of chronic low back pain.
Type III Modic changes are characterized by the presence of sclerosis or bony remodeling in the vertebral body. They are less common and may indicate a more advanced stage of degeneration.
Modic changes are typically associated with back pain, and the presence of specific types may suggest different underlying causes.
Vertebral Hemangioma:

Vertebral hemangioma is a benign tumor or vascular malformation that occurs within the vertebral body.
These lesions are usually asymptomatic and are often discovered incidentally on imaging studies, such as X-rays or MRI.
Vertebral hemangiomas are composed of blood vessels and may appear as a honeycomb or "corduroy" pattern on imaging.
While they are generally benign, in rare cases, they can cause symptoms such as pain or neurologic deficits if they compress nearby structures (spinal cord or nerve roots). This compression is more common in larger hemangiomas.
Treatment is usually not required unless the hemangioma is causing symptoms, in which case options may include surgery or other interventional procedures to alleviate pressure on the spinal cord or nerves.
In summary, Modic changes are alterations in the vertebral bone marrow seen on MRI and are associated with degenerative disc disease and back pain, while vertebral hemangiomas are benign vascular lesions within the vertebral body, often asymptomatic but potentially causing symptoms if they compress nearby structures. The two conditions have different causes, appearances on imaging, and clinical implications.
