Cure gum disease. Periodontitis is characterized by microbially-associated, host mediated inflammation that results in loss of periodontal attachment. While the periodontal microbiome is well described, its non-bacterial component needs a better understanding. Metagenomic analysis show a strong increase of the protozoan Entamoeba gingivalis in inflamed preriodontal pockets. A parasite is an organism that lives at the expense of its host. Can its manifest different levels of pathogenicity or remain an apparently symptom-free host? Despite good oral hygiene, the presence of E. gingivalis is associated with periodontal deterioration and pockets typically 3mm or more in depth. Quality Phase contrast microscopy offers a dynamic overview of the periodontal microbiota and allows to understand some virulent characteristic of the amoebae. Main characteristics are displacement toward specific environment within sub gingival plaque, channels formation within biofilm, apparent adhesion to human cells, deterioration of leukocyte and intra cellular granular activity, exonucleophagy trogocytic process avoiding first line of innate immunity, multiple vacuolar WBC nucleus digestion, MPN ghost cell creation, parasite nesting and reproducing within periodontal pocket and multiple pseudopodes trogocythosis. Those determinant abilities lead to evident contribution toward a pathological state. Exonucleophagy against the nuclear content of the neutrophil leads to alteration of the first line of defense cellular immunity and is thus unable to accomplish its normal function Understanding those functional characteristics of Entamoeba gingivalis during periodontitis contribute to define the amoebae as a pathogen.
