CC418. Class III Molar Protraction

Abstract
This case report describes the conservative treatment of a 32-year-old woman who was diagnosed of pseudo Class III malocclusion associated with anterior crossbite and bilateral mandibular first molar extraction sites. A fixed appliance in combination with posterior bite turbos and early light short elastics(ELSEs) were indicated to correct the anterior crossbite. The mandibular first molar extraction sites were closed by protraction of second molars and retraction of incisors with bilateral ELSEs and power chains. After 20 months of treatment, normal overjet and overbite were achieved and the profile with protrusive lower lip had been improved to an excellent cast-radiograph evaluation (CRE) of 25 points. No external root resorption or bony defect of mandibular second molars were noted, resulting in a restoration of occlusal functions. The patient was pleased with esthetic improvement in the smile arc. The Pink & White dental esthetics score is 3 points.

此病例為一名32歲女性，主要診斷為pseudo Class III malocclusion，合併前牙錯咬以及下顎雙側皆有缺牙空間。

保守的治療計畫是利用後牙墊高bite turbo加上第三類橡皮筋改善前牙錯咬，
下顎缺牙空間則是靠使用雙邊的橡皮筋以及power chains將第二大臼齒往前以及前牙往後移動來關閉空間。

經過20個月的治療，overjet和overbite達到正常值，原本過於前凸的下唇也獲得改善，術後評估CRE得分為25分。下顎缺牙空間關閉後，第二大臼齒無牙根或齒槽骨吸收的現象，咬合功能也恢復正常。病人很滿意治療結果，前牙美觀獲得改善，美觀評分為3分。
