Your doctor ordered the test, scan, procedure, or medication. You have health insurance. So why isn’t that enough?

In U.S. health insurance, some care requires prior authorization before it can happen. That means your insurance company reviews the request before agreeing to cover it. Until that prior authorization is approved, the pharmacy, imaging center, specialist, or hospital may not be able to move forward.

This short explains the difference between prior authorization and a claim denial. A claim denial usually happens after care is delivered. Prior authorization happens before care is delivered. The doctor says you need it, but the insurer says they need to verify it first.
