Open Right Hepatic Trisegmentectomy with “Hanging” Maneuver + Portal Vein Reconstruction

A 76-year-old male patient presented with right-sided Hepatocellular carcinoma. On CT imaging, the mass extended medially to segments 4A and 4B. Additionally, there was evidence of a tumor thrombus at the right posterior sectoral portal vein just nudging into the main portal vein.

This video discusses imaging and gives a play-by-play of an open right hepatic trisegmentectomy with a liver “hanging” maneuver and portal vein reconstruction. Postoperative course and pathology are also discussed.
