Biceps Palpation Rotation Test | Partial Distal Biceps Tendon Rupture

The Biceps Palpation Rotation Test is a focused physical exam maneuver used to evaluate partial distal biceps tendon ruptures. Pain reproduced with forearm pronation while palpating the radial tuberosity helps localize pathology at the distal biceps insertion.

The biceps palpation rotation test evaluates for partial distal biceps tendon ruptures. With the patient’s arm resting at their side and the elbow flexed to 90 degrees, the examiner palpates the radial head proximally and the radial tuberosity distally. While maintaining constant pressure over the radial head, the examiner then passively pronates and supinates the forearm. A positive finding is characterized by the absence of tenderness during supination with reproduction of pain or tenderness during pronation. Sensitivity as high as 97%
