CML Through the Decades: Recapping the Evolution of Chronic Myeloid Leukemia Treatment

In an in-depth interview with CancerNetwork®, Jorge Cortes, MD, director, and Cecil F. Whitaker Jr., GRA Eminent Scholar Chair in Cancer of the Georgia Cancer Center, provided a historical overview of the chronic myeloid leukemia (CML) treatment landscape, highlighting the dramatic shifts brought about by the development of tyrosine kinase inhibitors (TKIs). Prior to the year 2000, the standard of care was limited to interferon, often combined with low-dose cytarabine or transplantation. Neither option was ideal; interferon was highly toxic with a low response rate, and transplants were a serious consideration due to limitations in donor availability, age, and significant mortality risk.

According to Cortes, the introduction of imatinib was "groundbreaking" in that it revolutionized CML care. Its remarkable impact was evident even in phase 1 studies, with patients who were relapsed/refractory to previous treatments showing significant responses. The subsequent phase 3 IRIS trial (NCT00006343; NCT00333840), which compared imatinib (Gleevec) to interferon-alfa, unequivocally demonstrated its superiority in terms of better tolerance and superior responses, solidifying its role as the new frontline standard of care.

As clinicians gained experience with imatinib, resistance and intolerance emerged as key challenges, leading to the development of second-generation TKIs like dasatinib (Sprycel), nilotinib (Tasigna), and bosutinib (Bosulif). These drugs provided better results, even for patients who had not responded well to imatinib. Additionally, the discovery of the T315I mutation, which conferred resistance to all available drugs, sparked the development of the third-generation TKI, ponatinib (Iclusig).

Ponatinib proved to be highly potent but also introduced life-threatening arterial occlusive adverse effects, such as heart attacks and strokes. Cortes also mentioned more recent drugs with a different mechanism of action that continue to improve the treatment landscape, underscoring that while great progress has been made, there is still work to be done to improve quality of life and achieve a cure for all patients.
