🧠 Lecture: Multiple Sclerosis (MS)
In this NCLEX review, Dr. Atkinson breaks down Multiple Sclerosis — a chronic, immune-mediated neuro disorder every nursing student needs to know.
We’ll cover:
Overview & Risk Factors: Autoimmune demyelination, genetic + environmental influences, vitamin D, obesity, salt intake, and why MS clusters in northern regions.
Pathophysiology: T-cell mediated inflammation, myelin destruction, plaque formation, and axonal loss across the CNS.
Types of MS:
Relapsing–Remitting (most common)
Secondary Progressive
Primary Progressive
Progressive–Relapsing (least common)
Clinical Features: Fatigue, spasticity, pain, weakness, visual changes, tremors, ataxia, bladder/bowel issues, cognitive decline, depression.
Triggers & Relapses: Heat, illness, stress, deconditioning.
Older Adult Considerations: Polypharmacy, falls, immobility, infections, shortened life expectancy.
Diagnostics: MRI (lesions in space/time), CSF oligoclonal bands, evoked potentials.
Treatment:
Disease-modifying therapies (interferons, glatiramer acetate, oral agents, ocrelizumab).
Acute relapse management (high-dose IV steroids).
Symptom control: spasticity meds, fatigue agents, bladder + pain management.
Nursing Care: Fall prevention, mobility support, fatigue management, spasticity control, patient/family education, psychosocial support, depression screening, and transition planning.
