Hepatic Tuberculosis — The Great Mimicker Strikes the Liver!
TB affects the liver in three patterns: Miliary (most common), Nodular tuberculomas, Biliary (ductal) involvement.
Miliary Hepatic TB:
Multiple tiny granulomas throughout liver and spleen. Ultrasound: hepatomegaly, tiny hypoechoic nodules. CT: miliary hypodense nodules in both liver and spleen simultaneously — highly suggestive of TB.
Nodular TB (Tuberculomas):
Discrete 1-3cm nodules. CT: peripheral enhancement, central necrosis (caseous core).
Necrotic Porta Hepatis Lymphadenopathy:
PATHOGNOMONIC for TB. Central caseation, peripheral rim enhancement. Necrotic porta hepatis nodes equals TB until proven otherwise.
Biliary TB:
Porta hepatis nodes compress bile ducts — extrinsic biliary obstruction — post-hepatic jaundice. Mimics Klatskin tumour and cholangiocarcinoma.
Calcifications in liver and spleen with no symptoms equals healed TB — no treatment needed.
TB causes BOTH hepatic AND post-hepatic jaundice.
Radiology suggests diagnosis. Confirmation requires Mantoux, culture, liver biopsy (Langhans giant cells), IGRA.
