The Posterior Drawer Test is a classic physical examination maneuver used to evaluate the posterior cruciate ligament (PCL) and posterior knee instability. The posterior drawer test evaluates for PCL injuries. The patient lies supine with the hip and knee flexed while the foot rests in a neutral position. The examiner stabilizes the foot with their body, then grasps the proximal tibia with both hands positioned around the anterior and posterior aspects of the leg. A posteriorly directed force is applied to assess posterior translation of the tibia relative to the femur, comparing findings with the unaffected side. A positive test is indicated by excessive posterior tibial translation and/or the absence of a firm endpoint. Sensitivity is 90%, specificity is 99%
