Thalassaemia causes pre-hepatic jaundice from chronic haemolysis. But the radiological findings are unique and fascinating.
Bone Changes: Hair-on-end skull, expanded marrow spaces, cortical thinning, frontal bossing, vertebral deformities.
Extramedullary Haematopoiesis: Body makes blood outside bone marrow. Paravertebral masses on CT — not tumours!
Spleen and Liver: Massive splenomegaly, hepatomegaly from iron overload and extramedullary haematopoiesis. Normal bile ducts — non-obstructive jaundice.
Iron Overload from Transfusions: MRI T2 signal loss in liver and heart — iron deposition. Cardiac MRI T2 star for myocardial siderosis.
Pearl: Young patient with jaundice plus splenomegaly plus bone changes — think thalassaemia first!
