Analyzing The Clinical Data of 212Pb-DOTAMTATE in PRRT-Naive GEP-NETs
Mary Maluccio, MD, MPH, FACS, a professor of Surgery and chief of the Division of Surgical Oncology at Louisiana State University School of Medicine, as well as the Medical Director of the Louisiana Neuroendocrine Tumor Program, spoke with CancerNetwork® about her presentation on a peptide receptor radionuclide therapy (PRRT)–naive cohort of patients with gastroenteropancreatic neuroendocrine tumors (GEP-NETs) who received the investigational somatostatin receptor (SSTR)–targeted alpha therapy 212Pb-DOTAMTATE (AlphaMedixTM) at the 2025 North American Neuroendocrine Tumor Society (NANETS) Multidisciplinary NET Medical Symposium.
She explained that among patients who had never received prior radioligand therapy or who had received a beta particle radioligand, objective response rates (ORRs) surpassed 50%. For the PRRT-naive population, an ORR of 60% was observed, as well as a disease control rate of 94.3%, which Maluccio expressed as “unexpectedly good.”
Regarding safety, Maluccio asserted that the presentation’s question-and-answer session saw discussion of renal toxicities and dysphagia. For renal toxicities, she expressed that they are inherently associated with radioligand therapy, although no significant toxicities were observed with beta particles. She additionally explained that alpha particles may be more likely associated with renal toxicities in patients with elevated creatinine levels or among those who would ultimately develop kidney disease, with these toxicities emerging beyond the fourth treatment dose.
For dysphagia, she focused more on how to treat patients dealing with dysphagia rather than why it happens. She concluded by outlining methods for managing dysphagia, such as functional measures like botulinum neurotoxin (Botox) injections.
